Access to Damian OnLine

Who can use Damian OnLine?

The Damian OnLine app can be used by patients who:

  • are registered in the database of the Damian Medical Centre, i.e. who have a patient record with a unique MRN number in the Damian Medical Centre system (set up by the reception at one of the facilities or by registering online).

What can I do after logging into the simple version of Damian OnLine?

By logging into the simple version of Damian OnLine with your patient record number and PESEL, you can:

  • book appointments and diagnostic tests,
  • sign up to the reserve list,
  • pay for a booked appointment.

What can I do after logging into the full version of Damian OnLine?

In the full version, you get access to all functions. Log in using your patient record number (MRN) and password. In the full version you can:

  • book, cancel or reschedule appointments and examinations at Damian Medical Centres,
  • check the results (including laboratory test results displayed in graphical form), read comments on the results and medical recommendations,
  • order e-prescriptions that have been issued by your doctor, check and download e-prescription codes,
  • check referrals for tests and to specialists,
  • download your medical records: test results and consultation notes,
  • use the reserve list,
  • manage family profiles without having to log into different accounts,
  • submit a declaration on the choice of your primary healthcare facility (“POZ declaration”) (web version only),
  • use the calendar of vaccinations, screening examinations and NFZ schemes tailored to the patient’s gender and age,
  • pay for a booked appointment.

I am a foreigner, can I use Damian OnLine?

Yes, foreigners should provide the patient record number (MRN) and the number of their passport or other document used when registering the patient at the facility – this is how you can log into the simple version of the system. You need to obtain a password for the full version of Damian OnLine.

How do I get a password for Damian OnLine?

You can get your Damian OnLine password in two ways:

  1. Through a secure online banking system.

Log into the simple version via online24.damian.pl, you will be redirected to a list of banks. Select your bank and follow the instructions.

  1. At the reception of Damian Medical Centre

Bring your photo ID and ask for a first-time password.

If you are under 18, the password to your profile can be collected by your legal guardian at the reception of any Damian Medical Centre.

How do I change the password I received at Damian Medical Centre?

The first time you log in using the password collected from the reception of Damian Medical Centre, we will notify you about the need to change your password. Your new chosen password should consist of 12 characters and include:

  • a lower-case and upper-case letter,
  • a digit,
  • a special character (such as &, @, !).

I cannot remember my password or record number for Damian OnLine. How can I retrieve them?

Use the password/record number reminder option on the login page:

  • go to online24.damian.pl, click on “Forgot password” or “Forgot patient record number”,
  • enter the data required for a password reminder - patient record number, email address and date of birth, or the data required for a record number reminder - first name, last name, PESEL or passport number,
  • remember that the e-mail address you enter should match the one in the Damian Medical Centre database,
  • open the e-mail and follow the instructions.

Please note that the password reminder function will not work if you have never logged into the full version of Damian OnLine before.

After entering the wrong password, my account was blocked – what should I do?

After entering the wrong password three times, the Damian OnLine app gets blocked. Wait 15 minutes and try logging in again.

Can I get my password by telephone?

No, for security reasons the password is not provided by telephone. In order to obtain an individual access password and make full use of Damian OnLine, an adult can download the password via e-banking or report at the reception of any Damian Medical Centre in person with an identity document. In case of minors under 18, the profile password can be collected by their legal guardian.

Appointments

How can I book an appointment via Damian OnLine?

Log into the Damian OnLine app. After logging in, you will see a search engine. Select the relevant speciality group, e.g. NFZ Allergologist, Internist. The results will show all contact options for the speciality group searched. In the next step, available appointments will be presented together with information on the doctor’s speciality.

How does the speciality search engine work?

When you select a speciality group, you will see:

  1. Facility visits – in-person consultations or diagnostic tests.
  2. Quick consultations, among which you can choose:
  • Last Minute appointments – in-person appointment available on the day of the booking,
  • Telephone consultation arranged for a specific time.

Can I use Damian OnLine to cancel an appointment that was made via the Helpline?

Yes, if you access the full version of the app, you can cancel an appointment, even if it was not made through Damian OnLine.

Do cancelled appointments disappear from Damian OnLine as soon as they are cancelled?

Yes, once an appointment is cancelled, it immediately disappears from Damian OnLine.

How do I make an appointment for a diagnostic test?

You can book most diagnostic tests via the Damian OnLine app. For diagnostic tests that are not available on Damian OnLine, please contact the Call Centre on 22 566 22 22.

Can I make an appointment for a laboratory test via Damian OnLine?

No, laboratory tests are not available for booking via Damian OnLine. Contact the Call Centre to check the locations of the collection points and the range of tests performed.

Referrals and test results

Can I check in Damian OnLine what service I have a referral for?

Yes, when you log into the full version of Damian OnLine you will see the ‘Referrals’ section on the homepage. There you can check what service the doctor has referred you to, the referral expiry date and the status of the referral. You can also make an appointment straight away with your referral.

How can I print test results from Damian OnLine?

When you log into the full version of Damian OnLine, you will see the ‘Test Results’ section on the main page. Find the relevant test, click on the three dots next to the it and then click ‘Download’. You can download test results with the doctor’s impression in PDF format and, if necessary, print out medical documentation.

Will I be able to check all my test results on Damian OnLine?

No, not all test results are available on Damian OnLine, as some require additional medical interpretation or involve procedures that are not normally made available online.

How do I retrieve my medical records?

You will download your medical records by logging into the full version of Damian OnLine. Go to ‘Medical Records’. The maximum number of documents you can download is 10. Please note that in case of self-printed medical records, consistency of the copy with the original will not be certified. If you need your medical records for admission to hospital, etc., order them by calling the Call Centre on 22 566 22 22 and collect them in person at Damian Medical Centre.

e-Prescriptions

I want to check the code of an e-prescription issued by a doctor, how can I do this?

Log into the full version of the Damian OnLine app. Under e-prescriptions, you will find the date of the e-prescription, the code and QR code, as well as the medication prescribed. By clicking on the details, you can also check the recommended dosage.

What conditions do I have to fulfil to order an NFZ e-prescription from Damian OnLine?

You can order an NFZ e-prescription from Damian OnLine if:

  • you have an active POZ declaration,
  • the medicine has previously been prescribed by a NFZ internal medicine doctor or a NFZ paediatrician,
  • less than 12 months have passed since the last appointment during which the medicine was prescribed.

To order an NFZ e-prescription:

  • log into the full version of the Damian OnLine app using your patient record number and password,
  • after logging in, on the main page of the patient panel, you will see the ‘e-Prescriptions’ section, select the ‘Order Medication’ tab,
  • under the Orders tab - check the status of your order.

How will I know if an e-Prescription ordered from Damian OnLine has already been issued?

Once an e-Prescription has been issued:

  • you will receive an email with information on the change in the status of your e-Prescription,
  • you will see a notification of a change in the status of your e-Prescription order in the Damian OnLine app.

POZ declaration

Can I make a POZ declaration in Damian OnLine?

Yes, you can make a POZ declaration in the full version of Damian OnLine by logging into the portal via online24.damian.pl. Next steps:

  • select the POZ Declaration tab,
  • select a doctor, nurse and midwife, click - Submit Declaration,
  • indicate the purpose and place of declaration submission,
  • indicate which time this year you are submitting the declaration,
  • check the correctness of the data entered and sign the declaration with the Trusted Profile.

If the process was successful, you will receive an email confirming declaration submission.

Patient Profiles

What are Patient Profiles in Damian OnLine and what are they used for?

Profile linking allows you to link your profile with other profiles, such as those of your children or other relatives.

How can I share my profile with another user?

After logging into Damian OnLine in the computer version (online24.damian.pl):

  • in the top right-hand corner, go to the menu and click on your name,
  • select “manage profiles” and then “share profile”,
  • in the window, enter the record number of the user with whom you want to share your profile,
  • fill in the user’s PESEL number (if the person who is to manage your profile does not have a PESEL number, you can enter their date of birth),
  • after filling in the data, click “Send”,
  • you will receive a code needed to accept the linking of profiles,
  • we will send an e-mail to the e-mail address of the person you wish to share your profile with, asking them to accept the linking of your profiles,
  • in order to confirm that you agree to profile linking, the user who is to manage your account must enter the code that was displayed in Damian OnLine after the linking request was submitted,
  • once the user who is to manage your account has agreed to link their profile, they will be able to manage your health without having to log into your profile, for example to make or cancel appointments.

Can I view medical records in the linked profile?

No - the tabs: Medical Records, My Data and Settings are not accessible from the linked profile.

How do I delete a linked profile from my account?

To delete a profile linked with your account:

  • log into the Damian OnLine app, go to the “Manage Profiles” tab,
  • in the list of managed profiles, find the person whose profile you want to unlink from your account and then click the “Unlink” button.

We will confirm the unlinking of the patient’s account by email - we will send a message to the account owner and the unlinked person.

Whose profile can I link to my Damian OnLine account?

You can link any patient of Damian Medical Centre to your profile. You can manage, for example, the account of your child or loved one. All you need is your patient record number (MRN) and your PESEL number or, for those without a PESEL number, your date of birth.

What are the benefits of linking profiles in Damian OnLine?

By linking profiles, the person you share your account with will be able to perform the following actions on your behalf:

  • make and cancel appointments,
  • check test results,
  • order e-prescriptions,
  • view referrals.

How do I go back to my account after using another patient’s profile on Damian OnLine?

When using the Damian OnLine app on behalf of the person whose profile you have linked to your account, you will see this message at the top of the screen: “You are currently managing: Full name. Switch to your profile”.

To return to your profile, simply click on “Switch to your profile”.

How many patient profiles can I link to my Damian OnLine account?

You can link up to 10 other patient profiles to your account.

How can I check the status of my request to link profiles?

  • Once logged in, go to “Manage profiles”.
  • Once your linking request has been accepted or rejected, you will see the status of the request.

A child whose profile I have linked to my account has reached the age of eighteen. What happens now?

The child’s profile will be automatically unlinked from the legal guardian’s profile.

A person of legal age acquires legal capacity, so it is necessary to receive a new password for Damian OnLine and to share the profile again, if you so decide.

Reserve list

How do I use the reserve list at Damian OnLine?

If you have not been able to find a suitable appointment date for you, you can sign up for the reserve list on Damian OnLine or by calling the Call Centre on 22 566 22 22. Signing up for the reserve list at Damian OnLine takes a few steps:

  • below the appointment search results, you will find a link to sign up for the reserve list,
  • fill in the form and specify the required appointment - you can specify the desired date, the facility, the doctor and the speciality,
  • enter the telephone number at which you would like to receive text messages about your proposed appointment,
  • when you receive a text message informing you of a pending appointment proposal in Damian OnLine, let us know if it is suitable for you by logging into your profile using the link provided in the text message.

How many appointment proposals can I receive?

You will receive up to 5 appointment proposals and we will wait 30 minutes for your response.

Can I opt out of the reserve list?

Yes, you can opt out of your reserve list at any time via the Damian OnLine Patient Portal or in Damian Mobile, under My Appointments > Reserve List.

Payments

How can I pay for an appointment booked via Damian OnLine?

You can pay for an appointment booked via Damian OnLine in two convenient ways:

  • Using fast online payments - after pre-booking your appointment online, you can go directly from the order summary to the Przelewy24 system, where you can make a fast and secure payment. This is a convenient option that will allow you to pay your fee before your appointment. Once payment has been made, we will inform you if your payment has been successful and your appointment will be marked as paid.
  • At medical facility - you can make the payment directly at our facility before your appointment.

How do I get an invoice for a paid appointment?

You can request an invoice for your order via:

How can I get a refund for a paid invoice?

You can obtain a refund for a paid appointment by completing the form available on the website at https://www.sklep.damian.pl/zwroty within 14 days of purchase. Once the form has been submitted, the request will be processed and the funds will be refunded within 14 days to the indicated bank account.

Can I reschedule an appointment paid for on Damian Online?

Paid appointments can be rescheduled by using the ‘Change Appointment’ option - this function can be found in the appointment details.

Calendar of vaccinations and screening examinations

How to use the vaccination calendar?

Before using the calendar, you need to accept the terms and conditions of use of the calendar.

  • When you enter the calendar, the system will redirect you to the relevant age category.
  • The calendar shows the recommended vaccination doses that you can get and the vaccinations that you have already received at Damian Medical Centre (the system downloads data on completed vaccinations automatically).
  • You can set the frequency of notifications reminding you of your next doses.

What can I findin the screening tests calendar?

Suggested examinations and consultations appropriate to your gender and age, together with their frequency recommended by the medical team at Damian Medical Centre. 

Subsequent examinations are colour-coded and you can see how many tests have been completed and how many you complete in a given age range. Examinations completed at DMC will be added to the calendar automatically, appointments done at other facilities you can add yourself.

You will be reminded of your next screening check-up so that you can make an appointment in good time.

How can I change personal details in Damian OnLine?

You can change contact data after logging into the full version of Damian OnLine.

Once you have logged in, hover over your name in the top right-hand corner and then go to ‘My Details’. Remember to click ‘Save’ after making changes.

All changed information will be automatically saved in the Damian Medical Centre system.

Is it possible to check the price list of the Damian Medical Centre services in Damian OnLine?
